REQUIREMENTS - All Grades
Completed Application Form
Certified Copy of Birth Certificate
Copy of Standardized Testing Scorses and Grade Report Cards from previous school(s)
Immunization Records (All Immunizations MUST be current before the child can attend any classes) (T.B. TINE Test is REQUIRED for all New Students to start school.
Copy of any Current Court Order or Decree relating To the Custody/Convservatorship of this student.
Completed Emergency Medical Information
Copy of Sacramental Records if Catholic (Baptism, First Communion, Confirmation)

Parent/Guardian Involvement Form


GRADES - Pre-Kindergarten through the 8th Grade

All New Students MUST pay the following fees at the time of Registration:
Registration Fee – is Non-Refundable
Pre-K through 8th Grade Fees (Books, Supplies, Snacks)

IF ACCEPTED

To complete the registration process accepted applicants must pay the following required fees at the
School Office before, August, 1st
Parent Teacher Organization Fee
Family Contribution Contract Fee

TUITION

10 monthly payments beginning August 1st
Due the 1st of each month

REGISTRATION IS NOT COMPLETE UNTIL ALL REQUIREMENTS ARE FULFILLED
